*Intraepidermal vesicular dermatitis: Ballooning vesicular dermatitis [#b6a27213]

表皮内水疱を形成してくる病変のうち, &color(#c9171e){細胞内浮腫＝風船様変成(ballooning)が強くなり, 破れ融合して水疱を形成};したもの.

風船様変成は''通常表皮有棘層''に発生する.

風船様変成は%%%1個の細胞におこる変化%%%で, &color(#0000ff){細胞膜の破壊のない限り小水疱に相当する空隙をつくることはない};.-->病初期には風船様変成が存在したとしても, &color(#0000ff){水疱形成期にはこの変化は少ない};

網状変成(reticular degeneration): 細胞内浮腫の進行により細胞膜が破壊されると, 網の目様の物質の残存ができあがった水疱内に見られたり, 介在する残存細胞により均一な大きさのやや小型の空隙があつまり多房性の水疱を作ったりする状態

-実際に''水疱は風船様変成そのものではない''ので, むしろこの変化をreticulated vesicular dermatitisというほうが正しいのかもしれない.

ballooning vesicular dermatitisは真皮内浸潤細胞の局在により2つのタイプに亜分類される. これらの病変では同時に表皮下水疱形成をともなうものが多い.

>浅層のみに浸潤細胞の見られるsuperficial type

>浅層だけでなく深層にも浸潤細胞がおよぶ superficial and deep infiltrate
